ManualAuthenticator for SimpleHTTP.

Inspired by quite popular [1] letsencrypt-nosudo [2] by
@diafygi. Together with #440 and #473, it allows Let's Encrypt to be
used without sudo (root) on the target machine (c.f. [3]). Possibly
fixes #500.
